Medication Authorization Forms and Immunization Requirements
- View forms required for students to take medications at school. Your student’s doctor will need to sign all but the OTC form and a parent/guardian will need to sign all forms. This process can take up to a week with doctor offices so keep that in mind.
- View immunization requirements for the state of Ohio. If you have a kindergarten student starting this year, please make sure your student has had a four and five year check up with a doctor. Immunizations are completed at these appointments. Please turn in updated records to the school clinic.
- Please share any applicable health information with the Huber Ridge Clinic (614-797-7158).
